Information notices

Every form that collects personal data must tell the person, when they give it, who processes it, why, on what legal basis, who receives it and how long it is kept (articles 13 and 14 of the GDPR). MyZapp shows this information notice in the form itself, in a "How we use your personal data" box, and builds it from the records of processing.

The screen is under Data protection › Information notices. The whole module can read it; only data protection officers (and administrators) can change it.

Forms covered

Each module declares its collection forms: sign-up, invitation acceptance, public regulated-requests form, CRM web form and appointment booking, guest catalog order, assistant for visitors without an account. A disabled module declares none.

For each form, the screen shows the module, the processing activities that cover it and the current version of the notice. The Preview link opens the full notice, as the person sees it.

Where the content comes from

For each processing activity in force linked to the form, the notice shows its name, the data controller, the purposes, the legal basis, the recipients, the retention period and any transfers outside the European Union. It adds the data protection officer contact of each controller and reminds the person of their rights.

An activity that is only proposed does not appear: validate it in the records first. If no activity in force covers a form, the screen says so and nothing is shown to the person.

Your organization's introduction

For each active language you can add a short introduction (4,000 characters at most): context, link to your privacy policy… Without an introduction in the reader's language, nothing is added: no other language is shown instead.

Versions

The version of a notice increases as soon as what it shows changes: introduction edited, processing activity changed, added or removed. It stays the same while nothing changes. The form sends the code and version of the notice that was read: a module that collects consent keeps them as proof in the consent register.
